I understand the purpose and benefits as listed above and give permission for the above-named person to ride the van/bus and participate in church services or activities.
I understand this includes even injury resulting in death, which my child now has or which may arise in the future in connection with the activity or participation in any other associated activities.
I expressly agree that this release, waiver and indemnity agreement is intended to be broad and inclusive as permitted by the law of the State of Colorado.
I expressly agree that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect.
I understand that this release contains the entire agreement between the parties hereto, and the terms of this release are contractual and not a mere recital.
I also agree to binding arbitration developed by LBC, expressly waiving any and all rights in law and equity to bring any civil disagreement before a court of law.
The above-named child has my permission to be baptized at the church if they would like to do so.
In order to protect my child and others I promise to keep my child home if he or she contracts an infectious disease that could be transmitted in any manner to anyone.
It is the church's policy not to allow children to attend if they are ill with any infectious disease or fever.
We reserve the right to refuse to let a person ride the van/bus for disruptive or dangerous behavior.
I further state that I HAVE CAREFULLY READ THE FOREGOING RELEASE AND KNOW THE CONTENTS THEREOF, AND I SIGN THIS RELEASE AS MY OWN FREE ACT.
By signing my name electronically on this registration form, I agreeing that my electronic signature is the legal equivalent of my manual signature on this form.
